ci: expand OS matrix and add SQLAlchemy as core dependency

- Expanded matrix tests to include macOS and Python 3.13 without include overrides
- Added fail-fast: false to smoke install test
- Added macos-latest to smoke install matrix
- Moved sqlalchemy[asyncio] from optional extras to core dependencies
- Removed sqlalchemy from db, postgres, and mysql extras
- Added blank line between CI jobs
