Most likely an aggregate issue. They are unfortunately not the most reliable.
Try this. Change BlackHole to the primary device in the aggregate.

I’m experiencing persistent and unstable behavior when using BlackHole with Rekordbox 7 and OBS on macOS, and despite extensive troubleshooting I haven’t been able to get a reliable setup. I’m running on an Apple Silicon Mac with macOS Tahoe 26.2, using OBS 32.0.4 and Rekordbox 7.2.9.0320, together with a Pioneer DJ FLX4 controller. My goal is to capture the Rekordbox master output in OBS for both Twitch streaming and local recording, while monitoring correctly through the FLX4.
I installed the official BlackHole driver and tested both BlackHole 2ch and BlackHole 16ch. I created an Aggregate Device in Audio MIDI Setup combining the FLX4 and BlackHole, with the FLX4 set as the clock source, drift correction enabled on BlackHole, and the sample rate set to 44.1 kHz everywhere (system, aggregate device, Rekordbox, and OBS). In Rekordbox, I selected the aggregate device as the audio device and routed the master output to the FLX4. In OBS, I set the audio sample rate to 44.1 kHz, disabled Desktop Audio, and added BlackHole as an Audio Input Capture source.
However, OBS never receives audio from BlackHole in a reliable way, and in most cases does not receive any audio at all. At the same time, the Rekordbox master output works correctly on the speakers connected to the FLX4, which confirms that the master signal itself is being generated properly. Headphone monitoring on the FLX4 is unstable and inconsistent, with unpredictable behavior between CUE signals and the master signal, making it difficult to monitor correctly while testing.
The overall behavior is highly inconsistent and seems to vary depending on application launch order and configuration changes, making the setup unsuitable for live streaming. I observed essentially the same behavior when testing with BlackHole 16ch instead of BlackHole 2ch.
After many hours of troubleshooting, I tested the same setup using Loopback on the same machine, with the same versions of Rekordbox and OBS, and everything works perfectly and consistently. This suggests the issue may be related to BlackHole and/or aggregate device behavior with Rekordbox 7 on recent macOS versions and Apple Silicon.
I wanted to ask whether this is a known limitation or compatibility issue, and if there are any recommended settings or best practices when using BlackHole with DJ software like Rekordbox on modern macOS systems. Any insight or guidance would be greatly appreciated, and thank you for your work on BlackHole.

And without an aggregate? Directly into OBS?

Go back to your original setup that you told me about. While reproducing the issues in OBS capture the audio from BlackHole with Audacity.
Does the audio from Audacity sound fine?

Yes, I’ve also tried without an aggregate. When I set Rekordbox’s output directly to BlackHole, OBS captures the audio perfectly. However, in that setup I don’t hear anything at all in my FLX4 headphones or the connected speakers, so monitoring becomes impossible.

I just tried your suggestion. I went back to my original aggregate setup and tested capturing audio from BlackHole directly in Audacity. Unfortunately, Audacity does not capture any sound at all.

Fine. Then use any other daw. The goal here is to record BlackHole with a DAW at the same time as OBS.
