version 5.0.0.rc+9 Tax Group PHP Fatal Error On Add/Update Taxes

Note: I think this error does not break normal ops.
The app displays an incomplete page after the following:

1. Setup --> Tax Group Maintenance
2. Select a Tax Group and click Edit
3. Just click Update Order button --> results in the image below
4. Or click add from a selected Available Taxes and then click Update Order button --> results in the image below

I have no issues editing the name of an existing tax group using recent code [Main Menu > Setup > Tax Group Maintenance], click [Edit] for Ontario, edit name and click [Enter Group].

Fwiw, and imho, "Enter Group" implies I'm creating a new group, and "Save Group" would be more intuitive. Also a new group cannot be created from this page, which seems odd.

I have finally fixed this now. Gave me a headache trying to work the answer out, then realised it was actually a very easy answer!
