This adds issue templates and a PR template to the Code for Boston GitHub Organization. These templates will be applied to all projects that do not have their own templates defined.
The issue templates are based almost completely on the GitHub standard templates, with only small tweaks to trim templates down to what works for most Code for Boston teams. This is part of an effort to create default health files for Code for Boston's GitHub.
