Gitkraken MCP server in Gitlens breaks chat due to an `undefined` server name

Description

1. Install Gitlens and get the Gitkraken MCP server installed
2. Chat is broken with the error Cannot read properties of undefined (reading 'toLowerCase')

stil root causing what the actual source of this is, but this appears to have started about 12:30 PDT today

Of course an extension should ideally not be able to break chat like this, but a fix here would be lovely 🙏

We are fixing this ASAP and will get a patch release out.

But @connor4312 this is definitely something that VS Code should be verifying and not allow an extension to break VS Code like this.

In the meantime you can also set gitlens.gitkraken.mcp.autoEnabled to false and it will stop registering the MCP causing this.
