fix(codex): settle idle turns after missed completion events - #28029
A Codex thread can report idle while T3 still considers its turn active because turn/completed was missed. The adapter ignores idle status, leaving the run and its queued messages blocked. #28028 records the deterministic protocol replay and the limits of the original incident evidence.
Change
After an idle notification, allow the usual completion notification one second to arrive. If the exact context remains active, page through turn metadata with itemsView: notLoaded until that exact native turn is found. Settle only a matching terminal result through the existing completion and capacity-retry path. This also recovers an older goal turn when a newer orphan continuation appears first in history. The lookup runs outside the notification reader, shares one ten-second deadline across its pages, and stops if the context settles. Normal completions and capacity retries avoid the read.
Scope and approval
This is a focused reliability fix for the existing native-turn completion contract. It recovers an authoritative terminal state rather than inventing a completion result from inactivity. It changes only the Codex adapter and its existing tests. The companion issue supplies the failure evidence and proposed fix for maintainers to assess; it is not a claim of prior maintainer approval.
Verification
Added seven replay cases: completed, failed, interrupted, still in progress, a different latest turn, normal completion during the grace period, and completion racing the read. Six failed before implementation; all pass after it.
Added the review regression where an original goal turn loses its completion event and a newer orphan continuation is paused/interrupted. The original turn is on the next history page. This fails on 43ccc5b and passes with pagination; only the exact original turn is completed.
The full adapter suite passes all 130 tests, including capacity retry cases extended with idle notifications.
vp run check:changed upstream/main: format, lint, both knip passes, generated GraphQL check, affected typechecks, and 131 tests pass on Windows.
Actual adapter replay output with turn/completed omitted: running → thread/turns/list → completed → one terminal event.
Original upstream CI passed all checks on 43ccc5b. Its separate fork check run failed the untouched storageCleanup.test.ts case compares the checked-out branch without a full status: Git 2.56 reported gitfile does not point to a valid repository for the deliberately invalid worktree, producing failed instead of the expected kept. The Codex adapter tests, quality checks, and other fork jobs passed. Follow-up fork CI on d778fe3 is running.
Not verified with a naturally occurring lost notification or a live Codex model. The original long-running incident has insufficient retained diagnostics to establish that it emitted idle; this PR fixes the narrower reproduced state. A provider that remains genuinely in progress is left running.

Macroscope's review found this PR approvable — Recovers Codex turns left active after missed completion events by checking the provider’s terminal state after an idle notification. Matching-turn checks, bounded requests and existing serialized settlement keep the recovery narrowly scoped.

Walkthrough
CodexAdapterV2 now checks paginated native turn history when a thread reports idle. It routes a matching terminal turn through shared handling. Replay tests cover terminal statuses, turn mismatches, running turns, completion during reconciliation, and capacity-failure scenarios.
Terminal-turn processing moves to handleTerminalTurn. It retains status mapping, interruption adjustment, capacity-retry handling, and finalization.
Idle-thread reconciliation and coverage apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts
When a thread becomes idle, the adapter waits one second and pages through native turns. It stops when the context is inactive, the turn is absent or still running, or a request times out. Tests cover reconciliation outcomes, paginated history, completion during the read, and capacity-failure replay.
Issue 28028 describes an active Codex run remaining in a running state after an idle notification, with queued messages blocked. This change adds turn-history reconciliation for that behavior.

Explanation
The description follows the template sections (Problem, Change, Scope and approval, Verification). It explains the failure, the reconciliation approach, and the scope limits. Verification lists specific replay cases, test counts, and results, and it states what was not verified. The Problem section gives the expected behavior and a reproduction state, and it links the companion issue. The template's optional closing note about the agent model is present.
