Add support for Discord thread links & GM Forum thread links in resource headers

It would be nice to provide autocomplete links to GM specific discussion places. Currently, there is a couple dozen submissions that do have a thread link to Discord threads as we speak. (All in due part to Chef Bots amazing work)

To make this feature fully worthwhile, a majority of the submissions should be updated with thread links.
