The SM90 forward kernel already supports per-request dynamic_causal (an
int32 tensor, one entry per batch: 1=causal, 0=bidirectional), but the
SM100 kernel ignored it. Worse, since the shared interface threads
dynamic_causal positionally between seqused_k and page_table for all
arches while the SM100 __call__/kernel signatures had no such slot, every
paged SM100 call on this branch was positionally misaligned (the
dynamic_causal tensor landed in the mPageTable argument).

This adds mDynamicCausal to the SM100 __call__/kernel signatures at the
same positional slot as SM90, re-aligning the shared interface, and reads
the per-batch flag in each of the five warps (load, mma, softmax,
correction, epilogue). For a bidirectional sequence (psc == 0) each warp
recomputes [n_block_min, n_block_max) over the full key range -- and, for
split-KV, partitions it into disjoint per-split slices -- identically, so
the producer/consumer block counts agree (a mismatch deadlocks the
pipeline). The softmax warp's mask uses the existing AttentionMask
dynamic_causal field, extended here for apply_mask_sm100, to drop the
causal column clip for bidirectional rows. Mirrors the SM90 logic in
style (block_info.get_n_block_min_max stays untouched; the range
adjustment is inline in the kernel).

Also adds `dynamic_causal is not None` to the forward compile cache key:
a causal-compiled kernel with vs without a dynamic_causal tensor compiles
to different code (the const_expr branches are stripped when it is None),
so they must not share a cache entry.

The kernel must be compiled causal for the per-request switch to take
effect: psc == 1 entries keep the causal mask/range, psc == 0 entries
widen to bidirectional. Callers pass causal=True together with the
dynamic_causal tensor (same convention as SM90).
