fix/store owned delta chunk ids - #5920

The store-owned delta keyed its chunk-id map by the chunk's position inside the delta, but looked facts up by their position in the document. Any changed chunk other than the first wrote facts with no chunk id, so the next re-send could not replace them and they piled up. Remap through delta_chunk_map, as the Postgres delta path already does. Fixes #5919
